ABSTRACT

Background: Ferritin is one of the inflammatory markers used to predict the severity of COVID-19. Early assessment of severity is expected to be a priority in disease management. This study aims to determine the association between ferritin levels and the severity of confirmed COVID-19 patients at dr. M. Djamil Hospital Padang. Methods: This study is a retrospective cohort study on confirmed COVID-19 patients from January to May 2021. Chi-square analysis was calculated to assess the association between ferritin levels and clinical grade, the severity of the chest X-ray, and the level of need for oxygen therapy. To assess the risk opportunities for ferritin levels on each dependent variable, an association analysis was performed by calculating the Odds Ratio. Results: Characteristics of the patients were mainly female (54.25%), aged more than 50 years (59,00%), a clinical grade above category 4 (54,75%), required oxygen therapy (74,75%), the severity of chest X-ray was mild (75.50%), ferritin level <500 ng/ml (52.75%) and had no comorbidities (51.25%). This study found that ferritin levels correlated with a clinical grade, severity of chest X-ray, and level of need for oxygen therapy with HFNC and ventilator (P < 0.001). Ferritin levels >1000 ng/mL have a risk opportunity for clinical grade category 7 OR=8.28 (95% CI=2.69-25.41), severe chest X-ray severity OR=5.52 (95% CI=2.55-11.97), and need for oxygen therapy with HFNC and ventilator, OR=4.76 (95% CI=2.70-8.39) vs. OR=7.69 (95% CI=3.97-14.92). Conclusion: High ferritin levels significantly increase the risk of severe clinical severity, severe chest X-ray, and the level of need for oxygen therapy using HFNC and a ventilator in COVID-19 patients.

ABSTRACT

BACKGROUND: The Covid-19 outbreak is rapidly increasing as evidenced by the number of cases, deaths and countries affected. However, limited data is available on pregnant women with Covid-19 on which to base recommendations for pregnancy and post-partum care. AIM: Identify the effectiveness of nursing intervention with health education via an android application about self-care and prevention of Covid-19 transmission toward the knowledge and attitudes of pregnant, post-partum and lactating women. METHODS: This study was done in three stages. Stage one was identifying the existing nursing intervention for respondents with Covid-19, stage two involved the health education formulation and built it in to an android application, stage three measured the differences in knowledge attitude of respondents before and after health education by using the android application. 152 respondents pregnant, post-partum and lactating with Covid-19 women participated in this study. RESULTS: Stage one showed that nursing intervention has been done comprehensively. Stage two established the health education: http://cegahcovidbumil.000webhostapp.com;Stage three found that there were differences between pre and post health education via an android application in the knowledge and attitudes during pregnancy, post-partum and lactating women in self-care and prevention of Covid-19 transmission. Significancy was found among post-partum and lactating women (p=0.018*). CONCLUSION: There was a difference in knowledge and attitudes among respondents in self-care and prevention regarding the spread of Covid-19 before and after health education via an android application. Study recommended that health care providers use this method in their intervention to improve health status and prevent further spread of Covid-19. Further research to explore more health indicators is needed.

ABSTRACT

Since the COVID-19 outbreak began, healthcare providers have had to adapt how they triage, diagnose, and care for patients. Intensive care nurses are challenged by a new working scenario inside the COVID-19 Intensive Care Unit (ICU). They must provide the expected high standard care with advanced technology for patients while maintaining the important role of nurse’s therapeutic touch. This review discusses the touch and technology of nursing in general, which is correlated with providing nursing care in the Intensive Care Unit (ICU) during the pandemic. This study aimed to create theoretical foundations for nurses’ role in dealing with technology in the ICU during the COVID-19 pandemic. As a result, nurses appreciate the advantages of technology, but they also recognize that human interaction is important. Nurses must be able to provide appropriate care through the use of high technology without forgetting the value of nursing itself which is manifested in the form of therapeutic touch. © 2021 UPM Press.
